Pre-Surgical Registration

Prior to your procedure, please complete pre-surgical registration through your patient portal account. 

• Click HERE to create your Washington Regional MyChart patient portal account.
  
• If you receive an activation code by text or email, use your activation code to create your account. If you do not have an activation code, click "Sign up with your information". 
• Once you have created your patient portal account, please answer your pre-admission questions and review all instructions.
• Please allow 15 minutes to complete the documents.
• Once you have completed both the questionnaire and instructions, your portal will show each task as complete.
• Our staff will call if we have any questions.
• You will receive text or email reminders as your procedure date approaches.
