Enrollment Process
Step 1
Find out if you, a loved one, or a patient meets the basic requirements of the PACE of the Ozarks
program.
One of our enrollment specialists will provide preliminary information about eligibility and the
costs of the PACE of the Ozarks.
Step 2
Visit PACE of the Ozarks
Once it is determined that you meet the basic eligibility requirements, you will be invited to visit the PACE Center. If you need a ride, we can arrange transportation with one of our vehicles.
During this visit, you will tour the PACE Center, have lunch, and learn more about PACE of the Ozarks. IF you decide that you would like to enroll in our program, we will assist you in gathering medical records from your current care provider and arrange a home visit.
Step 3
Home Visit: Our nurse will visit your home
Following your visit to PACE of the Ozarks, a nurse and an enrollment coordinator will visit your home. They will ask questions about your current care needs, medical needs, and living situation in order to determine how PACE can assist you. This meeting will provide you and your love ones with information about the PACE program and give everyone the opportunity to ask questions to help decide if the PACE of the Ozarks program is the right choice.

Step 4
Assessment and Enrollment
For those who need Medicaid assistance to pay for their care, our enrollment coordinator will assist with an application. To qualify, you must meet financial and State of Arkansas level of care requirements. The State of Arkansas sends a nurse to your home to assess your care needs. This process takes time, but our enrollment coordinator will assist you with every step of the process.
Step 5
Your Individual Care Plan
Once enrolled we create an individualized care plan with you to ensure your needs are met by our team in a way that supports your independence.
