The concept behind PACE (Programs of All-inclusive Care of Elderly) is to provide comprehensive care for the seniors who live at home, but still need some services that they would otherwise receive from a Nursing Facility. Our resources are designed to help you maintain your quality of life without sacrificing your independence.

• A team of dedicated healthcare professionals monitoring your health status to reduce preventable and unnecessary hospitalizations
• Access to reliable transportation to medical appointments
• Medications delivered to your home
• Access to an adult day center for supervision and socialization up to 5 days a week as determined by the IDT
• Access to home care services, incontinence supplies, other medical equipment, and all approved IDT services
• Physical and occupational therapy to maintain level of functioning and prevent decline in abilities as approved by IDT
