Our Educators
Diabetes Management
Outpatient diabetes self-management education program for people of all ages with Type 1, Type 2, or gestational diabetes.
The program includes:
- Pre-class assessment by certified diabetes educator
- Individualized learning objectives and behavior goals for each patient
- Initial 10-hour program for newly diagnosed patients or those who have not received education
- Annual 2-hour education update class for patients who meet Medicare criteria
- Follow-up services including a 1-month appointment with diabetes educator
- Support group meetings
A provider referral is required for participation in the program. Self-pay and most insurances are accepted, including Medicare.

Diabetes Support Group
Meets the third Wednesday of every other month, May – November, from 5:00 – 6:00 p.m. at the William L. Bradley Medical Plaza (3 E. Appleby Rd, Suite 102, Fayetteville).
