Your Washington Regional Primary Care Clinic is Participating in a New Medicare Program to Enhance Your Care: Advanced Primary Care Management 

Advanced Primary Care Management (APCM) is an innovative program that ensures your primary care clinic continues to have the resources to meet your needs. 


Giving your care team extra support to help you get better care

Through APCM, we will receive additional resources from Medicare and other health insurance companies to help us enhance our work and provide you the best quality, patient-centered care.

Whether it's phone calls about results, reminders for screenings and annual visits, MyChart messages from your provider, or education about controlling your health, your primary care team does a lot of work behind the scenes. 

APCM helps our clinic continue to offer these services:
  • 24/7 access to your primary care team
  • Easy access to your medical records through MyChart
  • Telehealth and virtual visits
  • Support for the management of any chronic conditions
  • Personalized care plans for managing your health
  • Help after you leave the Emergency Department or hospital
  • Improved communication through MyChart

More information for patients with original Medicare

As part of this program, Medicare will start sharing some of your personal health information with us, such as when you receive care at hospitals, emergency departments, and specialist offices. This will help provide us with a more complete picture of your health and allow us to better coordinate your care.

If you want to stop Medicare from sharing this information, you should call 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048.

Your Medicare benefits are not changing, and this program is only intended to enhance the health care you receive at your Washington Regional primary care clinic. You still have the right to use or visit any doctor or hospital that accepts Medicare, at any time. Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it is always your choice which doctors you use or hospital you visit.
