Patients who take prescription medications are advised to adhere to the schedule of lab and office visits recommended by their doctor. However, if you need a refill between office visits, please call your pharmacy. No early refills for narcotics will be approved via telephone or portal request, an appointment must be made.

Click here to request a refill through the patient portal.

A medical request signed by the patient is required to send or release records to anyone. If you are requesting a copy of your records, there may be an additional fee.
