Refill Guidelines
Refills will be done Monday – Thursday.  Refills requested after 2pm will be done the next business day.  No early refills on pain medications or narcotics.  Refills should be requested from the pharmacy a minimum of 2-3 days prior to running out.  Refills will be called in within 48 hours.  Call your pharmacy after this time to see if refills are ready.  Patients need to be seen by the provider at appropriate intervals.  Please bring your medications or a list of your medications with you to your appointment.  If you need refills on any medications, ask your provider to write them during your appointment.  If you are taking critical medications, please adhere to your provider’s requirements for lab and office visits.  If you have a mail order pharmacy plan, you will need to pick up written prescriptions to mail in.

Appointments
You may schedule an appointment with one of our providers in several ways.  First, if you have never been seen by one of our providers before, ask your primary care physician to contact our scheduling office at 479-463-3000 to arrange for a new patient referral or they fax your medical records along with a referral form to 479-463-3050.  Once your primary care physician has sent the referral to our office one of our providers for review your information and our appointment coordinator will contact you with your appointment information.

If you have been seen by one of providers either in the hospital or in our clinic and need a follow-up visit,  please contact our appointment coordinator at 479-463-3000 for assistance.

For your first appointment please bring a photo ID and your insurance card along with a list of any medications your are taking, including over-the-counter medications.  You should also bring any medical records you can provide.  You must bring a copy of your current x-rays, MRI, or CT scans for review.  This can be either a CD or the actual films.  Although you should bring a copy of the reports, the report alone will not be adequate for your surgical consultation and planning. Please contact our office at 479-463-3000 if you do not have a copy of your films or studies prior to your visit so that we may assist you.

Financial Arrangements
Your insurance cards are requested at the time of you first visit.  Payments of co-pays, your portion of your remaining deductible are/or co-insurance is required at the time service is rendered.  Each time you have a medical visit, you are required to pay.  We accept cash, checks and credit (debit) cards.  Please contact our office prior to your appointment if you need to make payment arrangements.

Medical Records
A medical request signed by the patient is required to send or release records to anyone.  If you are requesting a copy of your records, there will be an additional fee.
