Telephone Calls
Telephone calls are answered between 8:00 am - 4:30 pm Monday - Friday. Please call your pharmacy when you need medication refills.

Refill Guidelines
Refills are done daily (advanced notice required). Refills requested after 2 pm will be done the next business day. No early refills on pain medications or narcotics. Refills should be requested from the pharmacy a minimum of 2-3 days prior to running out. Refills will be called in within 48 hours. Call your pharmacy after this time to see if refills are ready. Antibiotics are not refillable. You will need to make an appointment. Patients need to be seen by the provider at appropriate intervals. It is extremely important that you bring your medications, or a complete detailed list of your medications, with you to your appointment. If you need refills on any medications, ask your provider to write them during your appointment. If you are taking critical medications, please adhere to your provider's requirements for lab and office visits. If you have a mail order pharmacy plan, you will need to pick up written prescriptions to mail in.

Financial Arrangements
Your insurance card(s) will be requested at each visit. We are conservative in treatment to help limit the cost of medical care. Payments of co-pays, your portion of your remaining deductible and/or co-insurance is required at the time service is rendered. Each time you have a medical visit, you are required to pay. We accept cash, checks and credit (debit) cards.

We make every effort to see you in a timely manner. You may make an appointment by calling 479-463-2000. We request you call to reschedule or cancel if you are unable to keep your appointment time.

Medical Records
A medical request signed by the patient is required to send or release records to anyone. If you are requesting a copy of your records, there will be an additional fee. We use an outside copy service for our records and processing may take approximately two weeks.

We welcome you to call us at Fayetteville Family Clinic for an appointment at 463-2000 or 267-2235 between the hours of 8:00 am and 4:30 pm Monday - Friday. We appreciate your confidence in our quality health care.
