About our Team
Dr. Matthew Totten has been providing family medicine services in Northwest Arkansas since receiving his medical degree from University of Arkansas for Medical Sciences in 2003. Following medical school, Dr. Totten completed his residency training in family medicine at UAMS AHEC Northwest in Fayetteville and Springdale, where he served as Chief Resident. He is board-certified by the American Board of Family Medicine and is a member of the American Academy of Family Physicians. His professional interests include diabetes / chronic disease management as well as the use of computers and technology in medical practice. When not in clinic, Dr. Totten enjoys spending time with his wife and daughter plus traveling, fishing, and computers.
Shannon Jones A.P.N., N.P.-C., C.W.S., is a native of Northwest Arkansas. She is a graduate of the University of Arkansas as well as the University of Missouri – Kansas City, where she earned her master’s degree as a family nurse practitioner. In 2012 she also earned designation as a certified wound specialist, a national certification for wound care physicians and nurse practitioners. Shannon also works for Washington Regional as clinic director for the Wound Care Clinic, IV Infusion Clinic, Diabetic Education, Hyperbaric Oxygen and PICC Team of IV nurse specialists. Shannon and her husband have two sons. In addition to watching her sons participate in rodeo, Shannon also enjoys working on her farm, gardening and cooking.

Hours The Clinic is open Monday 9:00 a.m - 7:00 p.m., Tuesday through Thursday 8:00 a.m. - 4:30 p.m., and Friday 7:30 a.m. - 4:30 a.m.
Holidays We are closed New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving (Thursday and Friday), Christmas Eve and Christmas Day. If the clinic is to be closed, signs will be posted.
Inclement Weather Please check local television stations and/or their websites for announcements regarding clinic closures due to inclement weather. Patients should call the clinic before coming in during inclement weather.
Emergencies In case of an emergency, please go to Washington Regional Medical Center or the nearest hospital emergency room. If you have a medical problem and our clinic is not open, please call Washington Regional Medical Center at 479-463-1000. Ask the operator to page the physician on call.
Telephone Calls Telephone calls are answered between 7:30 am - 4:30 pm Monday - Friday. Please call your pharmacy when you need medication refills.
Refill Guidelines Patients who take critical prescription medications are advised to adhere to the schedule of lab and office visits recommended by their doctor. Please bring your medications, or a list of them, to your office visit and ask your doctor to write refills for any medications you anticipate needing. Patients who have been seen by the doctor at appropriate intervals may call for refills on some prescriptions (antibiotics and narcotics excluded). You may call your pharmacy 2 - 3 days prior to running out of medication so the pharmacy can fax a refill request to the doctor. Please give 48 hours notice for refill requests. Requests received after 2 p.m. will be processed within the next two business days. Refills are done on Monday, Tuesday, Wednesday and Thursday. Patients who use a mail order pharmacy plan will need to pick up written prescriptions from the clinic and mail them to their providing pharmacy.
Financial Arrangements Your insurance card(s) will be requested at each visit. We are conservative in treatment to help limit the cost of medical care. Payments of co-pays, your portion of your remaining deductible and/or co-insurance is required at the time service is rendered. Each time you have a medical visit, you are required to pay. We accept cash, checks and credit (debit) cards.
Appointments You may make an appointment by calling (479) 463-4887. To request an appointment online, click here. We request you call to reschedule if you are unable to keep your appointment so that we may offer that appointment time to another patient. We make every effort to see you in a timely manner including seeing you as a walk-in and same day services.
Medical Records A medical release of information form signed by the patient is required to send records to anyone including other doctors participating in your care. We are happy to assist you with obtaining your medical records from other clinics or hospitals. If you have received medical care in a hospital, emergency department, surgery center, or specialty clinic since your last appointment in our clinic, please notify us so we can make sure that those records are available at the time of your next appointment and any follow up care needed prior to that next appointment can be provided. If you request a copy of your records from our clinic, you will be charged a fee to copy them.
New Patients If you are a new patient to our clinic, please fill out the new patient questionnaire and bring it with you to your first appointment. When you call for your appointment, you will be asked if you currently see any other doctors, have had any recent tests, or have been recently treated in a hospital or emergency department. If you have had any of these medical services, we will request your permission to obtain your most recent records of these services. Please bring all of your medications with you to your appointment. If you are taking narcotics for chronic pain, please notify our receptionist BEFORE you schedule your new patient appointment. Our goal is to obtain a thorough medical history in order to provide comprehensive primary medical care that is tailored to you, your health, and your life.
Patient Documents
Laboratory Testing If you are scheduled to have fasting lab work, please do not eat after midnight prior to your test. We request that you drink only water between midnight and the time of your test. If you take medications, take all of your medications as usual the morning of the laboratory test unless you are diabetic. If you are diabetic, take all of your medications as usual the morning of the lab test EXCEPT YOUR DIABETES MEDICATIONS. Please bring your diabetes medications and a snack with you to your laboratory appointment so that you can eat and take your medications immediately following the testing. We try to schedule fasting lab work as early in the morning as possible to minimize the duration of your fast. If you have a history of hypoglycemia / low blood sugar when fasting or if you have a history of passing out when you have your blood drawn, please notify both the nurse and laboratory technician BEFORE you have your blood drawn so that precautions can be taken for your safety.
