Client Agreement
- It is my understanding that Faith in Action is an organization that provides volunteers to transport me to physician appointments, grocery store and essential errands, perform yard work, chore assistance, friendly visits, telephone visits and minor home repairs.
- I understand that if I need a service I will request the service by calling 479-463-7862 in Washington County and 479-273-3570 in Benton County at least five (5) days in advance of my need.
- I understand that all services are to be requested through the Faith in Action office, not the volunteer who is providing me service.
- I understand no payment will be accepted for the service provided.
- I also understand volunteers will not provide their personal phone number and volunteers will not witness legal documents or accept medication or care orders from physicians.
- I understand that if I need transportation I will wear a seat belt and I will not smoke in the automobile of the volunteer.
- I understand that if local schools are closed because of snow and ice there will be no volunteer services.
- I will advise the Faith in Action office if I will be using a cane, walker, wheelchair or have a need for additional assistance with my ambulation.
- I will provide an emergency contact name to the Faith in Action office and I will allow the FIA staff to confirm my appointment times and dates for me.
- I also understand that Faith in Action may discontinue my participation in Faith in Action at anytime and for any reason the Faith in Action team deems appropriate.
- I agree to follow the FIA guidelines for volunteer services.
