Overview
Our mission is to promote healthy aging by keeping older adults well, active and independent for as long as possible. Because our clinic is devoted exclusively to the service of older adults, our staff and services are uniquely sensitive to their needs and abilities. We are able to distinguish between the normal changes caused by aging and changes caused by disease. Illnesses that linger are an unfortunate accompaniment of old age, but sometimes older people with multiple chronic health problems assume they have to cope with continued symptoms and disability. A comprehensive geriatric assessment can determine what conditions are present and determine if increased or different care could help patients improve. With this approach, the best care can be identified.

Our Team
Our multidisciplinary team includes geriatricians, advanced practice nurses, counselors, social workers, and a geriatric neuropsychologist, neurologist, urologist, geriatric psychiatrist and nursing staff. All members have specialized training in geriatrics. You will be treated with kindness and compassion by a devoted team.

Your First Visit

• Complete geriatric assessment

• Involvement with own plan of care

Ongoing Primary Care

• Follow-up appointments based on plan of care
• Same day appointments with APNs for acute illnesses
• Educational activities
• Support Groups
• Community resource referral
• Coordination of care with other specialists

Senior Specialty Unit

• A new unit in WRMC, fully integrated with our Senior Health Clinic to ensure you receive the specialized care needed for acute illnesses requiring hospitalization.
• The unit features a comfortable, home-like atmosphere.
• An interdisciplinary team works with you to keep you physically and socially active.
• This approach helps you to recover faster and more easily regain your independence.
