You are requested to bring your insurance and/or Medicare cards on your first visit so we may file your visit. We make every effort to limit the cost of your medical care; therefore we request payment at the time of your visit. Payment of any of the following will help eliminate the cost of a monthly statement: paying your co-pay, or your 20%, or a deductible at the beginning of each year. We do require payment each time you have services rendered at the clinic. We accept cash, check, or credit cards.
